Whether it’s a sudden earthquake, prolonged conflict, or devastating flood, affected populations face immediate threats to their nutritional well-being. Ensuring that disaster survivors receive adequate, safe, and appropriate nutrition is fundamental to saving lives and supporting recovery. The Sphere Humanitarian Standards provide the globally recognized framework that guides humanitarian organizations in meeting these critical nutritional needs.

Why nutrition matters in disaster response
Disasters disrupt regular food supply chains, destroy stored food and livestock, and force people into temporary shelters where access to basic necessities becomes precarious. According to research published in the BMJ Open, the prevalence of acute malnutrition during emergencies can reach 10-15%, significantly higher than normal conditions. Children under five, pregnant and breastfeeding women, elderly individuals, and people with disabilities face heightened vulnerability during these crises.
Nutritional deficiencies don’t just cause hunger-they increase susceptibility to infections, slow wound healing, impair cognitive function, and can lead to death. The UNHCR’s emergency protocols emphasize that addressing acute malnutrition and micronutrient deficiencies has a significant impact on the health, well-being, and overall survival of affected populations.
Nutrient supply and food quality
Meeting the nutritional needs of disaster-affected populations requires more than simply providing calories. The minimum standards established by international humanitarian organizations specify that general food rations should contain at least 2,100 kilocalories of energy per person per day, along with adequate protein, fat, and essential micronutrients.
Essential components of emergency rations
A balanced emergency food basket must include diverse food sources to prevent malnutrition and deficiency diseases. The Turkish Journal of Emergency Medicine notes that key indicators for adequate nutrition include the absence of scurvy, pellagra, or beriberi-conditions that arise from specific vitamin deficiencies. This means food distributions must go beyond staples like rice or wheat to include sources of vitamins A, C, and B-complex nutrients.
Micronutrient-rich diets are particularly important for vulnerable groups. Children require adequate iron, zinc, and vitamin A for proper growth and immune function. Food commodities must be inspected regularly, and unfit items certified and disposed of according to standard procedures.
At the household level, affected populations need access to safe food preparation and storage facilities. This includes clean water for cooking, appropriate cooking utensils, and fuel-efficient stoves. The USDA’s disaster food assistance protocols emphasize the importance of maintaining the cold chain for perishable items and ensuring that distribution points meet basic sanitation requirements.
Cultural preferences and dietary requirements
Food acceptability goes beyond safety. Research on cultural factors in disaster response demonstrates that the selection of local and traditional foods is key to the success of nutrition interventions. The humanitarian standard specifies that foods provided must be appropriate and acceptable to the population-meaning consultation with affected communities about food preferences is essential.
Religious dietary laws must be strictly respected. During disaster response, ensuring the availability of halal, kosher, vegetarian, or other religiously mandated foods can be challenging but remains non-negotiable. As one humanitarian organization notes, the belief that hungry people will eat anything is simply untrue-food that conflicts with deeply held beliefs is unlikely to be consumed, regardless of hunger levels.
The type of staple food matters significantly. Different populations prefer different varieties of rice or types of lentils. Providing unfamiliar foods, even if nutritionally adequate, can lead to waste and nutritional deficits. Successful programs source familiar, locally appropriate foods whenever possible, treating food provision as an opportunity to respect the dignity of affected individuals.
Specialized nutritional support
While general food distributions meet the needs of most disaster-affected individuals, certain groups require specialized nutritional interventions to survive and recover.
Managing acute malnutrition
Acute malnutrition represents a severe health condition characterized by rapid deterioration in nutritional status. The Community-Based Management of Acute Malnutrition (CMAM) approach has become the standard for treating malnourished individuals in emergencies. This model involves systematic screening using anthropometric measurements, particularly mid-upper arm circumference (MUAC) and weight-for-height ratios, to identify and classify cases by severity.
Severe acute malnutrition (SAM) requires immediate intervention. Cases with medical complications need inpatient stabilization, where therapeutic feeding, medical treatment, and monitoring are provided. The WHO protocols specify that children with complicated SAM should be kept in warm areas (25-30ยฐC), with facilities and staff available for correct preparation of therapeutic foods and regular feeding day and night.
For SAM cases without medical complications, outpatient therapeutic care has proven highly effective. The cornerstone of this treatment is Ready-to-Use Therapeutic Food (RUTF)-an energy-dense, micronutrient-enriched paste typically made from peanuts, oil, sugar, and milk powder. UNICEF reports that RUTF has transformed the treatment of severe malnutrition, allowing children to recover at home with regular follow-up visits rather than requiring prolonged hospitalization.
Supplementary feeding programs
Moderate acute malnutrition (MAM) is addressed through supplementary feeding programs that provide additional nutritious foods on top of general rations. These programs typically use ready-to-use supplementary food (RUSF) or fortified blended foods like corn-soy blend. The target is to achieve recovery rates above 75% while keeping default rates below 15%.
Blanket supplementary feeding becomes necessary when malnutrition rates exceed 10-15% in the presence of aggravating factors such as disease outbreaks or inadequate general rations. These programs target all members of vulnerable groups-typically children under five and pregnant or breastfeeding women-rather than only those already malnourished.
Infant and young child feeding in emergencies
Infants and young children face the highest mortality risk during emergencies, making appropriate feeding practices literally lifesaving. The UNHCR maintains targets of 85% for timely initiation of breastfeeding and 75% for exclusive breastfeeding under six months. Achieving these targets requires establishing baby-friendly spaces at reception centers and within community structures, providing skilled breastfeeding counselling, and protecting against inappropriate donations of infant formula.
The International Code of Marketing of Breastmilk Substitutes guides emergency response to prevent well-meaning but harmful donations of powdered infant formula. Such donations have been shown to displace breastfeeding and increase infant mortality. When formula is genuinely needed for infants who cannot be breastfed, it must be provided as part of a sustained, coordinated care package with proper support for safe preparation and feeding.
For infants over six months, ensuring access to appropriate complementary foods becomes crucial. These foods must be nutritionally adequate, safely prepared, and culturally acceptable. Programs should also address the nutritional needs of pregnant and breastfeeding mothers, whose dietary requirements increase significantly during these life stages.
Coordination and monitoring
Effective nutrition response requires strong coordination among multiple partners-governments, UN agencies like UNICEF and the World Food Programme, and implementing NGOs. The combined protocol approach being piloted in several countries aims to streamline the treatment of both moderate and severe malnutrition at the same location, potentially improving coverage and outcomes.
Regular monitoring through standardized surveys and health information systems allows programs to track coverage, quality, and treatment outcomes. When malnutrition management shows recovery rates below 75% or default rates above 15%, programs must analyze barriers and adjust their approaches accordingly.
